If your child is in severe distress or unresponsive, please call 911 or come in to the office immediately.

Please review the steps below:

  1. Please stay in your car
  2. Please complete the registration form by clicking the button below.
  3. You will receive a text message confirming that you have been successfully added.
  4. A Nurse will reach out to you with next steps.
  5. If you do not receive a text reply please call 970-494-2626 or come into the office.

Effective January 2026 -Our Urgent Care will no longer perform Sport Physicals.

If Your Child is in Severe Distress and/or Non-Responsive please call 911 or Come In To Our Office and let us know it's an Emergency. To be seen at either the Youth Clinic or the Pediatric Urgent Care please fill out the form outside in your vehicle. Once the form is submitted, you will receive a confirmation text with a link to the waitlist. If you do not receive a confirmation text within 5 minuets of submitting the form, call the Pediatric Urgent Care phone number (970) 494-2626 or walk in and talk to the front desk to make sure you got on the waitlist. A nurse will either call or text you once they are ready for you to come inside. Thank you.
